Steps:
- Resolve context (explicit > env > PWD >
personal). - Slugify the name: lowercase, spaces → hyphens, strip non-
[a-z0-9-]. - Compute path:
$SOFIA_VAULT/_agent/plans/<context>/<slug>.md - If the file does not exist, create it with this template:
