Usage

Well-formed XML to stdout including:

Common edge cases
hrefvalues use forward slashes even on Windows (DITA convention)<keydef>keysattribute is derived from the filename stem (e.g.,intro.dita→key-intro)<reltable>is scaffolded but empty; remove it if not needed
