In Conductor worktrees, "Index is stale" can be an overlay/canonical-checkout mismatch. Use
scripts/codeindex-health.sh; only runscripts/codeindex-health.sh --repairwhen it reports the shared base index is stale. Verify branch-local code with source files/diffs.
