   - Sharp pain to cold, < 30s
   - No radiograph change
   - Treatment: remove etiology + monitor
2. Irreversible pulpitis

   - Treatment: pulpectomy / extraction
3. Acute apical abscess
   - Severe spontaneous, percussion +ve
   - Possible swelling + lymph
   - Treatment: drainage, RCT or extraction

7. Trigeminal neuralgia

Red flags (refer immediately):
- Facial cellulitis with airway compromise — ER
